Clean Claim

A claim submitted without errors or missing information that a payer can adjudicate and pay without requesting anything further.

A clean claim is one that contains every required data element, codes the services correctly, and matches the provider's information on file, so the payer can process it on the first pass without follow-up. Stale or mismatched enrollment records are a frequent reason claims are rejected or pended, which is why accurate payer enrollment and disciplined provider data management directly improve clean-claim rates. Keeping a provider listed as a current participating provider helps payments arrive faster and reduces costly rework.
