Fee Schedule

The set of contracted rates a health plan agrees to pay a participating provider for covered services.

A fee schedule lists the dollar amounts a payer will reimburse for specific procedures or services, usually tied to billing codes. These rates are negotiated as part of the contract a participating provider signs during payer enrollment, and they determine what the practice ultimately collects on a covered claim. Understanding the applicable fee schedule helps providers forecast revenue and confirm that paid claims match what the contract promised.
