Certificate of Insurance (COI)

A Certificate of Insurance, or COI, is the summary document a carrier issues to confirm that a provider carries professional liability coverage. It typically lists the named insured, the per-claim and aggregate coverage limits, and the policy's effective and expiration dates. Credentialing teams collect a current COI as proof of active malpractice insurance, and they track expiration dates closely so coverage never lapses during a provider's appointment. When a provider switches carriers, the COI is also reviewed alongside any tail coverage to confirm there are no gaps.
